Q: casodex how long will i have to take it and how often do i get hormone injections?
A: Casodex is for the treatment of stage D2 metastatic carcinoma of the prostate. You will have to take it all the time unless you have problems with your liver. Then the doctor should have you stop.. Normally hormone shots are given once a month. You might consider getting an Orchectomy done(Removal of the testicles.) This is a permanent hormone treatment, hence no shots. I had that done. No big deal.
Q: with the drug Casodex, why should i as a female wear gloves and a mask?
A: Bicalutamide sold as Casodex is contraindicated in females and children. "contraindication is a condition or factor that increases the risks involved in using a particular drug". Adverse reactions include reproductive system and breast disorders, breast tenderness, gynaecomastia (breasts in men), hot flushes, gastrointestinal disorders, diarrhea, nausea, hepatic changes (elevated levels of transaminases, jaundice), asthenia and pruritus.
Gloves and masks prevent unintended absorption and inhalation.
Q: Is the Casodex antiandrogen effect permanent?
Prostate Cancer Treatment
A: No. Generally it is quite temporary, unfortunately so in fact. In my experience patients with metastatic prostate cancer generally respond for several months to Casodex, then may respond again when this agent is withdrawn, usually for less time than the initial response. It can treat the cancer when it is first given; changes in the cancer cells over time then make them susceptible to the withdrawal of the drug later.
Q: SIDE AFFECTS OF CASODEX?
would casodex cause double vision. or what else might cause it.
A: why did you place this in the respiratory disease category?
Q: what are drug substitutes for Casodex?
A: There are other anti androgen drugs like spironolactone, ketaconozole, and finasteride. Are you having too many side effects? Most drug substitutes of the same nature have the same side effects. Why exactly are you trying to inhibit your testosterone levels?

Q: when would you give a patient with spinal cord compression and PSA of 2000 ketoconazole?
patient with multiple bony mets, heme/onc already started on casodex and lupron (simultaneously) family refuses orchiectomy
A: Orchiectomy is the fastest way to obtain a castrate level of testosterone. Ketoconazole is the next fastest. If a patient has symptomatic spinal cord compression from prostate cancer mets, immediate orchiectomy or ketoconazole AND steroid bolus is indicated. Lupron will eventually result in chemical castration, but first causes a "flare" of testosterone that can initially worsen cord compression, but this affect is minimized by blocking the testosterone receptor with bicalutamide (casodex) Also, external beam radiation to the affected vertebrae can slow progress of mets and should be considered in a patient like this. Hope this helps.
